Eddie's House Case Management program targets at- risk and homeless veterans within a specific area; Orleans, Jefferson, and Kenner regions of Louisiana.
Clients go through a five stages of engagement in the Case Management services:
_ Assessment, Clients are prioritize based on urgency of housing crisis. Clients at greater risk of becoming homeless are addressed first. Clients discretion is adhered to achieve housing stability and wellness. Initial assessments are done in a comfortable environment. Eddie's House employ a diverse staff of case managers to use a variety of approaches to address clients needs.
_ Evaluation, Case managers review initial assessment to gauge the severity of the housing need. Case managers use crisis resolution and triage based on personal safety and community resources availability.
Eddie's House uses " client choice, respect, and empowerment model" in all instances where clients are being serviced. The generalist approach to ensure the help is found through partnering organizations is used. This avoids duplicate services in our region.
_ Plan, Eddie's House case managers develop a plan for the veteran to lift them into sustainable and permanent housing.
_ Implementation, Case managers working with each client to implement the plan agreed upon in partnership with Case managers, clients, resources , and partnering organizations. Each client won't specifically have the same plan, but all will work to the goal of permanent housing and self reliance.
_Analysis, Case managers will do a comprehensive review of clients' progress towards the goal set by carful planning and client involvement. The plan moves clients closer to obtaining permanent housing and lifting them to a place of self reliance. This is when adjustments or corrections will be implemented.
Case management objective is to go from Assessment to targeted resources to permanent housing and self reliance
